Just follow the steps.......
You are applying for a child passport.
Child Passport
All children under the age of 16 must follow the instructions for Child Passport, including any of the following:
  
  * Child applying for a U.S. Passport for the first time.
  * Child whose previous U.S. Passport has expired.
  * Child whose previous U.S. Passport has been lost or stolen.
  * Child whose name has been changed through court order              from the time last U.S. passport was issued.
When completed "Do not signyou will be directed to sign by acceptance agent.
U.S. Child Passport Application
Note: If your child was issued a U.S. Passport in the past and can't find it, in addition to DS-11 you must complete Form DS64
If the child is 16 years of age or younger, both parents and child must appear before the Acceptance Agent to sign the passport application. If only one parent appears other parent must sign and notarize a Statement of Consent Form DS-3053. It is also recommended that the absent parent provide a photocopy of  his or her valid state-issued ID.

 Proof of Identity of the Parent(s)
Parents must provide1 Photo copy of Their drivers license or state ID, with an issue date of 6 months or longer.Click here for Secondary Evidence of Idenitifaction
Provide one of the following; Certified U.S. Birth Certificate or your most recent U.S. Passport or your Naturalization documents.
Note: Mutilated, laminated, altered or damaged passports are not acceptable evidence of U.S. Citizenship. A hospital issued birth certificate WILL NOT be accepted.

If you are driving to Mexico or Canada, please provide a typed formal letter addresssed to the passport office explaining your dates of travel, means of  transportation, the border where you will be crossing, place of destination, and purpose of travel.
Business Travelers must provide letter from employer outlining immediate travel.
2 Copies of your plane ticket or official travel itinerary showing your name and date of travel.

Check or Money Order in the amount of $140.00 Made Payable To "U.S. Department Of State".
In order to verify your identity and maintain security when applying for a U.S. Passport, all passport applicants are required to show up in person at their local passport acceptance agent. These agents are located at Post Offices and County Clerk's Offices across the country. There will be a $25.00 fee.
