2 Passport photos
Your most recent passport is the last U.S.Passport issued by the passport office ( Issued in the last 15 yrs and not damaged) This passport will be returned with the new one.
Expired Passport
If your name has changed and you are currently using a different name than the one on your previous passport you must provide an original name change document bearing the official seal of the issuing authority.
Name Change
1 Copy of your plane ticket or official travel itinerary showing your name and date of travel.
Business Travelers must provide letter from employer outlining immediate travel.
If you are driving to Mexico or Canada, please provide a typed formal letter addressed to the passport office explaining your dates of travel, means of transportation, the border where you will be crossing, place of destination, and purpose of travel.
Evidence of Travel
Check or Money Order in the amount of $208.32 Made Payable To "U.S. Department Of State".
